グアム島のALSの本体に関する神経病理学的再検討

The Essential Feature of the Neuropathological Findings in ALS of Guam

はじめに

　グアム島在住Chamorro人には，かつて世界平均の数十倍の頻度で筋萎縮性側索硬化症（amyotrophic lateral sclerosis：ALS）が発症しており^1－8），parkinsonism-dementia complex（PDC，パーキンソン認知症)^9，10）もほぼ同時期に高頻度で発症していたという。グアム島のALSの発症のピークは1945～1965年頃であり，PDCのそれは1950～1970年頃と言われている。当時のannual incidence rateは，ALSでは10万人あたり170～40程度，PDCでは60～20程度で，ALSのannual incidence rateは集落ごとに250から0までの開きがあったという。ALSの発症率は1970年以降急激に減少し，1990年頃には世界平均とほぼ同等となり^8），2007年の現在は，グアム島（人口およそ16万人）にただ1人，のALS患者が生存中であるという^11）。一方，PDCは2000年頃も10万人あたり25～10程度を保ったままであるという^8）。

　グアム島のALSとは何であったのか？　1960年代に主張されたように，グアム島のALSは，ほぼ同時期にグアム島で多発していたPDCと同一の疾患であり，孤発性ALSとは異なる疾患である^12，13）のか？　あるいは筆者らや英国のグループがこの十数年来主張してきたように，グアム島のALSは，そもそも孤発性ALSであり，PDCとは，少なくとも神経病理学的には異なる疾患である^14－18）のか，これらの疑問についての今日的解析が本稿に与えられたテーマであろう。

　すなわち本稿では，

(1)グアム島のALSとグアム島以外のALSとの異同，

(2)グアム島のALSとグアムPDCとの異同，

(3)グアム島と紀伊半島のALSとPDC，

(4)グアム島のALS，PDC研究の意義，

などについて論じていく。

　本稿での解析のもととなるグアム症例は，合衆国国立衛生研究所（NIH）との共同研究として筆者らが1979年から1982年までグアム島に滞在して剖検，検索した計175例（ALS：11例，ALS-PDC：5例，PDC：50例，その他：109例）である。これらの所見に基づき，文献的考察を併せ加えて論を進める。

Abstract

　The amyotrophic lateral sclerosis (ALS) and parkinsonism-dementia complex (PDC) were the fatal neurological diseases, showing very high incidence during 1945―1965 and dramatic decrease after 1970 on Guam.　Based on the findings obtained, it is proposed that (a) NFTs in ALS of Guam are merely a background feature widely distributed in this population, (b) Guam ALS occurs initially as classic ALS, (c) ALS-PDC mixed patient is a case combined with classic ALS and neurofibrillary degeneration, (d) thus a subtype of "Guam ALS" is not present, (e) PDC and ALS of Guam are basically different diseases, and (f) PDC is a discrete disease entity.
