　患者は71歳，男性。8年前に器質性勃起障害のためセミリジッド型陰茎プロステーシスの移植を受けた。肉眼的血尿が出現し精査を施行したところ，膀胱右側壁に有茎性表在性腫瘍を認め，全身麻酔下に経尿道的膀胱腫瘍切除術を施行した。本症例における手術は特に支障はなかったが，腫瘍の存在位置によっては腫瘍切除の困難が予測された。その対策として，術前の腫瘍の評価とともに全長の長いリゼクトスコープを用意したり，会陰部からの膀胱鏡の挿入，また，陰茎を捻転させて尿道の距離を短縮させるなどの工夫が必要と考えられた。

A 71-year-old man with the semirigid penile prosthesis for erectile dysfunction 8-year before noticed a gross hematuria and visited our hospital. The ultrasonography revealed a bladder tumor at the right bladder wall. The transurethral resection of the bladder tumor was done. In our patient there was no problem in the transurethral resection procedure but several problems should be in mind in the endoscopic management of bladder tumor in patients with penile prosthesis, mainly the possible insufficient length of the endoscope.
