小児泌尿器科領域における日帰り手術の有用性

THE SIGNIFICANCE OF DAY SURGERY IN PEDIATRIC UROLOGY

　1985年5月から1996年1月の間に行われた日帰り手術患者1,168例を対象とし，合併症を中心に検討をした。麻酔関連合併症はなく，手術関連合併症は縫合糸膿瘍22例．術後出血6例，尿道留置カテーテル抜去1例であった。そのうち術後出血1例，カテーテル抜去1例に経過観察入院を要した。合併率は2.5％であった。合併症に対して外科的処置を要した症例はなく，術後合併症の頻度としては十分受容できると思われた。また，母子分離の影響の軽減や医療費の節約という点から考えても，小児泌尿器科領域において日帰り手術は有用である。今後の課題として手術適応の拡大があげられる。

1168 operations were performed mainly for cryptorchidism during May 1985, to January 1996 on a day surgery basis. We found no anesthetic-related complication, but 22 cases of stitch string abscess, 6 cases of hematoma, and 1 case of spontaneous removal of Foley catheter. Overall complication rate was 2.5%. One hematoma and one removal of Foley catheter required hospitalization for close observation. But no child required postoperative surgical procedure. Outpatient surgery is advantageous in decreasing stress of parent-child separation and cost of medical care.
