日帰り手術での尿道下裂術後尿道皮膚瘻閉鎖術

SIMPLE MULTILAYERED CLOSURE OF URETHROCUTANEOUS FISTULA ON AN OUTPATIENT BASIS

　尿道下裂修復術後に生じたピンホールから直径5mm程度の尿道皮膚瘻14例に対して日帰り手術を行い，術後の尿道カテーテルも留置せず全例閉鎖に成功したのでその手術手技を報告する。手技上のポイントとしては，瘻孔の基部まで十分に剥離し，瘻孔を完全に切除すること，閉鎖にはマイクロサージェリーの技術を応用し，繊細な縫合テクニックを用いること，切除部の皮膚面を確実に尿道内腔に入れるように縫合することなどが重要と考えられた。また，術後軽度の排尿痛を訴えることがあったが，排尿には問題なく，それ以外に特別な問題には遭遇しなかった。

The procedures of simple multilayered closure of urethrocutaneous fistula with microsurgical technique are described. Fourteen cases, who had undergone hypospadias repair previously, underwent this procedure on an outpatient basis and without urinary drainage postoperatively. All fistulas were successfully closed without any problem. The key points for successful closure are seemed to excise fistula completely and to keep meticulous suturing technique. Outpatient surgery is seemed beneficial to reduce the psychological and physical problem.
