　58歳，男性．主訴は心窩部痛と食欲不振．諸検査の結果，左副腎腫瘍と診断した．ホルモン学的には非活性であったが，癌胎児性抗原の異常高値を認めたため，転移性副腎腫瘍も考え精査するも原発巣は発見できなかった．左副腎摘出術を施行，病理組織学的診断は副腎皮質癌であった．術後，癌胎児性抗原が低下したこと，腫瘍組織が抗癌胎児性抗体で強く染色されたことより癌胎児性抗原産生腫瘍と考えられた．

A case of CEA producing adrenocortical adenocarcinoma is reported. A 58-year-old man was admittedto Hiroshima City Hospital complaining of anorexia and epigastralgia. A left adrenal tumor measuring 5×8cm was found. No endocrinological abnormalities were detected by laboratory examination. But the levelof serum CEA showed 134ng/ml. Left adrenalectomy was done and the histological diagnosis was adrenocor-tical adenocarcinoma. Postoperatively serum CEA decreased. The carcinoma cells were strongly positive forCEA by immunohistochemical staining.
