動脈尿管瘻の2例

TWO CASES OF ARTERIO-URETERAL FISTURA

　症例1は60歳，男性．S状結腸癌のため骨盤内臓器全摘出術を施行．術後48日目に回腸導管より動脈性出血を認め，感染による総腸骨動脈左尿管瘻と診断した．動脈穿孔部の縫合術を施行するも，139日目に死亡した．症例2は73歳，男性．膀胱腫瘍のため膀胱全摘出術を施行．術後1年，10回目の尿管カテーテル交換時に動脈性出血を認め，大動脈左尿管瘻と診断した．パッチ縫合による大動脈瘻閉鎖術を施行するも，12日目に死亡した．

Case 1 : A 60-year-old male underwent pelvic exentration for sigmoid cancer. 48 days after the operation, massive arterial bleeding occurred from the stoma of the ileal conduit, which was caused by left-common ileac arterio-left ureteral fistula due to infection. The fistula was sutured but the patient died 139 days after the operation due to multiple organ failure. Case 2 : A 73-year-old male underwent total cystectomy for bladder cancer. 1 year after the operation, at the 10th change of ureteral catheter, the patient was admmited due to arterial bleeding from the stoma of left-ureterocutaneostomy.
