経尿道的前立腺切除術の手術成績

TREATMENT OF PROSTATIC HYPERTROPHY BY TRANSURETHRAL RESECTION

緒言

　最近，前立腺肥大症に対して前立腺TURが一般的に高く評価され，年々施行される施設が多くなつてきている。しかし，前立腺TURは術者の技術の巧拙によつて大きく影響されるため患者に対しても術者の技術向上がぜひ必要になつてくる。このためには多数の経験を集中的に行う必要がある。筆者は1人医長として出張し，1人で麻酔から術中術後管理を行い，4年間に112例の前立腺TURを経験した。術中および術後トラブルをできるだけ少なくする工夫を加えて行つたので報告する。

TUR-P was performed in 112 cases of enlarged prostates at Kameda General Hospital from 1977 to 1980. All cases were done under saddle-block anesthesia with a drop infusion of 15% mannitol and 10% fructose with ephedrine hydrochloridum 40mg during the operation that helped for the stability of blood pressure. Three-way Foley catheter was remained in situ as bladder drainage catheter for 72 hours after the operations, first 24 hours with 400g weight.The average weight of resected prostates was 21.1g.
