胃癌の陰囊内転移

A case of gastric cancer with intrascrotal cancer cell dissemination

　症例は76歳，男性。無痛性の右陰囊内腫瘤のため紹介された。来院時，腫瘤は鶏卵大，弾性硬で可動性に乏しく，右精索に連続的に進展していた。右精巣腫瘍の診断で右高位精巣摘除術を施行した。病理組織診断は腺癌で，転移性陰囊内腫瘍と診断された。原発巣として消化器癌が考えられた。その後の検索により，びまん性腹膜播種を伴うⅢ型の進行胃癌が発見された。本症例における転移機序としては，胃癌腹膜播種からの連続性進展が考えられた。

　A 76-year-old man presented with a painless right intrascrotal tumor. The tumor was hen's-egg sized and hard without mobility,spreading directly to the spermatic cord. The patient underwent inguinal orchiectomy. The histopathological diagnosis was adenocarcinoma,presumably metastasized from the gastrointestinal tract. Advanced gastric cancer,TypeⅢ,with diffusely disseminated peritoneal lesions was found. The direct cancer cell dissemination from the peritoneal lesions through the tunica vaginalis was thought to be a possible metastatic pathway in this case.
