  • Abstract
  • Look Inside

A case of necrobiosis lipoidica induced by contusions was reported. The patient was 48-year-old woman who noticed oval reddish plaques on her both shins about 3 years ago. They gradually spread to periphery developing into scleroderma - like- plaques having teleangiectasia and well-defined and slight elevated borders. The center became depressed and sulfur-yellow in color and on some part of center changed into ulcer. The patient had no diabetes in her past and family history.

Laboratory finding ; urinanalysis were normal, but total cholesterol in serum was 216mg/dl and prediabetic data in standard and predonisolone glucose tolerance test were gained.

Histologic findings showed necrobiotic foci of collagen, sudanophilic granules between degenerated fibers, granulomatous infiltrate consisting of giant cells, epithelioid cells and lymphocytes and endothelial proliferation of small vessels with the thickening of the wall.

After treatment of Flucort cream in ODT method durling 2 months and half, lesions almostly improved.

A review of this disease was performed, especially on relation to diabetes. We asserted that on non-diabetic case, it is necessary to do predonisolone glucose tolerance test to deny diabetes correctly.
