A 79-year-old man presented with right sudden hearing loss and vertigo. At the first stage he showed no other neurological symptoms. Neuro-otological examination after admission revealeddirection-changing geotropic nystagmus, gaze-evok-ed nystagmus, saccadic pursuit, and decreased visual suppression of caloric nystagmus with de-creased caloric responses on the right. Combined lesions of the peripheral and central cochleo-ves-tibular system were suggested. MRI revealed infarc-tion in the antero-inferior area of the right cerebel-lum. Finally he was diagnosed as anterior inferior cerebellar artery (AICA) syndrome. As AICA syn-drome may present with mimic symptoms to the peripheral disease, we should be careful for diagno-sis.
