急性内耳障害症状で発症した前下小脳動脈症候群の1症例

A Case of Anterior Inferior Cerebellar Artery Syndrome Presenting the Mimic Symptoms to the Peripheral Disease

　はじめに

　急性感音難聴とめまいを主訴に来院する患者は多く，その多くは突発性難聴やメニエール病といった末梢性障害であるが，稀に小脳・脳幹梗塞といった中枢障害が混在することがある。

　今回われわれは，急性感音難聴とめまいを主訴に来院し，初診時に他の脳神経症状を認めず，突発性難聴に酷似した前下小脳動脈症候群の1例を経験したので報告する。

A 79-year-old man presented with right sudden hearing loss and vertigo. At the first stage he showed no other neurological symptoms. Neuro-otological examination after admission revealeddirection-changing geotropic nystagmus, gaze-evok-ed nystagmus, saccadic pursuit, and decreased visual suppression of caloric nystagmus with de-creased caloric responses on the right. Combined lesions of the peripheral and central cochleo-ves-tibular system were suggested. MRI revealed infarc-tion in the antero-inferior area of the right cerebel-lum. Finally he was diagnosed as anterior inferior cerebellar artery (AICA) syndrome. As AICA syn-drome may present with mimic symptoms to the peripheral disease, we should be careful for diagno-sis.
