稀な頭頸部結核症の2例—乳児症例と口腔癌治療後発症例

Two Cases of Rare Tuberculosis of Head and Neck Region:An Infant Case and a Case Occurring after Treatment of Oral Carcinoma

　緒言

　結核患者の発生は戦後，予防接種の普及ならびに国民の食生活，衛生管理の向上により急激に減少し現在の患者数は約24万人と30年前に比べ約7分の1となっている。しかし結核症は今日でも年間5万3千人に罹患し3千人以上が死亡するという最大の感染症であることに変わりはない。また近年では悪性腫瘍の治療中または治療後に結核症を併発する症例も報告され，また新たな問題となっている。今回私達は初発が生後10ヵ月という乳児と頬粘膜癌手術後2年3ヵ月後に発症した頭頸部にのみ限局した結核症の2例に遭遇したのでその概要を報告する。

We present two cases of rare tuberculosis (TB) of head and neck region, which were an infant with lymphadenitis and a patient of parotid TB occurring after treatment of buccal carcinoma.

The first case was supposed to be infected from mother. In the second case, the cause of TB was suspected as the decrease of immunity due to chemotherapy and steroids used for con-trol nausea from CDDP. And AO plate was sus-pected to be a cause of infection.

We discussed the difficulty of diagnosis of TB of head and neck region which occurred after treatment of oral carcinoma.
