小脳梗塞による急性感音難聴2例に対するステロイド治療

The steroid treatment for sudden hearing loss associated with cerebellar infarction

Ⅰ　はじめに

　急性感音難聴は日常診療で，しばしば遭遇する疾患であるが，時に脳血管障害が原因で発症する例のあることが知られている^1～3）。その発生頻度は1.2％と低いが^1），中枢神経症状に乏しい症例もあるため注意を要する。今回われわれは小脳梗塞による急性感音難聴の2症例を治療する機会を得たので，その臨床経過を述べるとともに若干の文献的考察を加えて報告する。

　We describe two patients with acute sensorineural hearing loss accompanied with cerebellar infarction. The first patient was a 67-year-old man with hypertension and gastric ulcer,and the second patient is an 80-year-old woman with hypertension,diabetes,and cerebral infarction. Both patients complained of vertigo,facial nerve paralysis of the right side and sensorineural hearing loss on the affected side. MRI depicted cerebellar infarction in the anterior inferior cerebellar artery（AICA）area. Hearing loss and facial nerve paralysis were improved by both antithrombus therapy and steroid treatment,suggesting that the steroid treatment might work to recover hearing if it is used together with antithrombus therapy.
