先天緑内障を伴ったcutis marmorata telangiectatica congenitaの1例

A case of cutis marmorata telangiectatica congenita associated with congenital glaucoma

　症例は生後10日の女児。出生時から特徴的な皮斑がありcutis marmorata telangi—ectatica congenita （以下CMTC）と診断され，眼科的精査のため当科を受診した。初診時，顔面を含む全身に血管腫がみられた。眼圧は右29，左32mmHgで，両眼とも角膜は浮腫状であり，角膜径は11mm×12mmと増大していた。隅角には多数の虹彩突起と虹彩の高位付着がみられ，シュレム管の充血も著明であった。両眼に2回のトラベクロトミーを行い，現在は眼圧は良好である。眼圧上昇機序として，隅角の形成異常だけでなく，上強膜血管圧の関与も考えられる。CMTCで顔面に血管腫がみられる場合，先天緑内障を合併することがあり，眼科的な経過観察が必要である。

A 10-day-old female baby was diagnosed as cutismarmorata telangiectatica congenita and referredto us for eye examinations. The whole body wascovered by generalized cutaneous angioma. Theintraocular pressure was 29 mmHg right and 32mmHg left. Both corneas were edematous andmeasured 11 mm×12 mm across. Gonioscopyshowed high iris insertion, numerous iris processesand blood filling of the Schlemm's canal.Trabeculotomy induced normalization of theintraocular pressure. It was presumed that thecongenital glaucoma was the result of increasedepiscleral venous pressure and abnormal chamberangle.
