膿疱性乾癬に網膜血管炎を合併した2例

Retinal vasuculitis in two cases of psoriasis pustulosa

（P−126）　46歳男性と25歳女性が，片眼霧視で受診した。両症例とも全身皮膚に小膿疱を伴う紅斑があり，皮膚科で膿疱性乾癬と診断されていた。両症例とも，霧視がある眼の視神経乳頭周辺に軟性白斑が散在していた。眼科的に無治療のまま，1例では約2週，他の1例では6週後に軟性白斑は消失した。軟性白斑の原因は，網膜血管炎による網膜血管の局所的閉塞であると推定した。膿疱性乾癬の症例では，定期的な眼底検査が望まれる。

Two patients, a 46-year-old male and 25-year-old female, presented with unilateral blurring of vision. Both had disseminated ethythema and had been diagnosed, dermatologically, as psoriasis pustulosa. Both cases showed soft exudates around the optic disc in the affected eyes. The soft exudates spontaneously disappeared after 2 and 6 weeks each. We presumed focal retinal ischemia secondary to retinal vasculitis as the cause of soft exudates. These cases illustrate the necessity of periodic fundus examinations in patients of psoriasis pustulosa.
