Abstract Purpose:To report a case of acute anterior uveitis and optic disc swelling post vaccination of mRNA-1273 for COVID-19.

Case and findings:A 17-year-old woman presented with blurred vision, injection, and pain, in the left eye continuing from the day after the first vaccination of mRNA-1273. Visual acuity was(1.5)and(1.0p)in the right and left eye, respectively. The left eye showed ciliary injection, anterior chamber inflammation with cells, flare, and fine keratic precipitates, and optic disc swelling. Upon diagnosis of anterior uveitis and optic disc swelling, steroid and atropine eye drops were administered. Shortly thereafter, her symptoms and signs except for optic disc swelling were gone. The day after the second vaccination of mRNA-1273, she had a fever, injection of both eyes, and blurred vision in the left eye. The left eye showed mild anterior chamber inflammation, considerable vitreous inflammation with cells and flare, and optic disc swelling. She was treated with the same regimen, and her symptoms and signs except for optic disc swelling recovered. The patient is currently being followed up without treatment.

Conclusion:We reported a case of acute anterior uveitis and optic disc swelling related with COVID-19 vaccine mRNA-1273.
