眼瞼結膜に認められたマントル細胞リンパ腫の1例

A case of mantle cell lymphoma in the palpebral conjunctiva

要約　目的：眼瞼結膜に認められたマントル細胞リンパ腫の1例の報告。

症例：67歳，男性。右下眼瞼結膜に約25mm大の隆起した腫瘤と円蓋部にやや隆起した表面平滑なピンク色の腫瘤があり，精査加療目的で近医から紹介され受診した。

経過：右下円蓋結膜腫瘍を生検し，病理組織診断，フローサイトメトリーにてCD5陽性，CD20陰性，cyclin D1の発現を認め，マントル細胞リンパ腫（MCL）と診断した。PET-CTで，右下眼瞼以外に，骨髄，前立腺，頸部に集積を認め，骨髄生検でMCLが検出され，stage Ⅳと病期診断した。右眼病変に対し放射線療法（18回計36Gy）を施行し，腫瘍は消失した。以後3年間再発はない。

結論：眼瞼結膜に発症したMCLに対し，放射線療法が奏効した。

Abstract　Purpose：To report a case of mantle cell lymphoma（MCL）in the conjunctiva of the eyelid.

Case：A 67-year-old man. Approximately 25 mm large tumor was found in the right lower eyelid and referral was received from a nearby doctor for the purpose of examination and treatment.

Observation：Removal of lower right fornix conjunctival tumor. Histopathology and flowcytometry showed mantle cell lymphoma（MCL）with expression of CD5 positive and cyclin D1. In addition to the lower right eyelid, tumor cell-like accumulation in bone marrow, prostate, cervix was confirmed by PET-CT. MCL was histologically detected in bone marrow biopsy sample, indicating the patient on the stage Ⅳ. Radiotherapy（18 times total 36 Gy）was performed on the right lower eyelid tumor and the tumor disappeared without recurrence during the observation interval.

Conclusion：We experienced a case of MLC in the palpebral conjunctiva. Radiation was effective for the conjunctival lesion.
