急性間質性腎炎に伴ったぶどう膜炎（Renal-ocular syndrome）

A case of uveitis with acute interstitial nephritis

　急性間質性腎炎（以下AIN）にぶどう膜炎を合併した42歳女性の例を経験した．本症ははじめに虹彩炎に気づかれ，次いで腎炎を発症し生検にてAINと確定診断された．そして，経過中に腎炎の再燃とは関連なくぶどう膜炎の再燃をみた．再燃時の眼所見としては前部ぶどう膜炎のほか軟性白斑，乳頭発赤腫脹，眼底出血，硝子体混濁等の後部ぶどう膜炎の所見も呈し，ステロイドの全身投与により改善した．これまでの報告例と比較，検討するとAINとぶどう膜炎の合併例は女性に好発し，前部ぶどう膜炎が必発してその多くが両眼性であること，腎炎とぶどう膜炎の活動性が一致しないこと，高IgG血症がみられ全身ステロイド療法が奏効すること等いくつかの共通点を有することがわかった．眼科領域では両者の合併はほとんど知られておらず，今後注目すべき症候群であると思われた．

A 12-year-old female presented with bilateral iriclocyclitis of 2 weeks' duration. Acute interstitial nephritis was diagnosed through renal biopsy. The nephritis showed prompt and favorable response to systemic corticosteroid therapy. The uveitis, which included optic disc edema, retinal hemorrhage, soft exudates and vitreous opacity. followed a protracted course until the inflammation subsided after 6 months of topical and systemic corticosteroid theratpy.

We reviewed this 'renal- ocular syndrome' in 9 cases from literature. This syndrome was characterized by a high incidence in young females, bilateral uveitis which was usually of anterior type, dissociation in the period of activity between nephritis and uveitis, and favorable response to systemic corticosteroid treatment.
