術後角膜内皮障害を認めたIOL挿入眼に関する考察

Corneal endothelial damage following intraocular lens implantation

　人工水晶体挿入後2〜3年で角膜内皮障害を認めた4症例を経時的に観察し以下の結果を得た。（1）　specular microscopeにより最後に観察しえた角膜内皮のaveragecell densityは400〜500／mm²，　average cell areaは2,000〜2,500μm²であった。（2）症例2および症例3のフルオ前房水角膜移行係数（kc・ac）は各々0.23, 1.49（hr⁻¹），分配係数（rac）は各々0.83, 0.74の値を得た。（3）全層角膜移植を行った2症例の被移植片および摘出人工水晶体の走査電顕的検索では露出したデスメ膜の網状線維性構造と島状に残存した角膜内皮の細胞間接合部の破壊像を認めた。また人工水晶体のレンズ表面に細胞の付着像は認めなかった。

We evaluated four cases which developed lasting corneal edema 2 to 3 years after intraocular lens im-plantation. Two of the cases developed severe bul-lous keratopathy and were treated by penetrating keratoplasty. The other two cases still retained use-ful vision in spite of partial bullous keratopathy.

We performed measurement of the corneal thick-ness and specular microscopy of the corneal endo-thelium in each case
