外傷性眼瞼下垂に対する治療

Treatment for Traumatic Blepharoptosis

緒言

　交通事故や労務災害にあつては，身体の露出部分である顔面に傷害を蒙ることが多く，そのさい眼部にもまた大きな損傷を受けることが多い。眼瞼裂傷にあつては併存する上眼瞼挙筋断裂に深い注意が払われず，特に外科医はその点に対する無知から単に皮膚創の縫合のみが行なわれ，創傷治癒後，完全な眼瞼下垂がのこり，眼科医へ送られてくることが多い。著者らは最近約3カ年間にこのような外傷性眼瞼下垂の6症例を経験し，二次的に断裂した上眼瞼挙筋の縫合を行ない，幸いにして全例にほぼ満足する結果を得たので，その成績について報告し，いささか考察を加えてなたい。

Surgical repair of six cases of traumatic ble-pharoptosis was described, in which severed levator palpabrase superioris muscle were su-tured to the tarsus 4 to 60 days after the first-aid operation and almost satisfactory results were obtained in all cases. The most appro-priate surgical approach is the transcutaneous approach rather than the 13lascovics' trans-conjunctival one. In cases associated with artificial anophthalmos, various degrees of lag-ophthalmos were observed after the operation due to fibrous adhesion of the muscle to intra-orbital tissues.
