Treatment of Corneal Ulcers with Cysteine Collyrium

緒言

　角膜潰瘍の発生や進行にコラゲナーゼが関与することがわかり，その阻害剤の研究も進んで，本症の治療法は近年著しく進歩した。

　多数のコラゲナーゼ阻害剤のうち，効果の確かさと刺激性や安全性などを考え合わせて，われわれは臨床応用にはシステインを最良と考え，実際これを角膜ヘルペスに点眼して十分な効果を収めることができた^15）。ところがその唯一の欠点は，水溶液がはなはだ不安定なことである。そのため作りおきがきかず，投与のつど新調する。その上ふつうは隔日，夏季には毎日作りかえねばならない。これはまことに煩わしい。

Although cysteine is a safe and potent colla-genase inhibitor there are some difficulties in the clinical application because of its extreme instability in aqueous solution. Addition of re-duced ascorbic acid and chondroitin sulfate, ho-wever, proved to increase its stability and effe-ctiveness, and to produce a composite collyrium for corneal ulcers. The results obtained with this remedy were :

1. Infective ulcers-Herpetic keratitis which had failed to respond to IDU therapy, zoster den-drite, vaccinial and EKC ulcers were all treated successfully. Among these the most striking effect was observed in cases of vaccinial ulcers.
