外直筋麻痺に対する筋移植後，合併症を起こした2症例

Two Cases with Postoperative Complications Following Extensive Muscle Transplantation

緒言

　1907年にHummelscheim^1）が外直筋麻痺に対し，上下直筋の外側1／3を麻痺筋付着部の上下へ移植する方法を提唱して以来，麻痺性斜視に対する筋移植術は多数行なわれてきている。本邦においても，中島実氏^2），須田経宇氏^3）らの報告があり，1965年には，近藤正彦氏^4）の35例の報告がある。国内，国外を通じて，最も報告の多い麻痺は外直筋麻痺の症例で，手術法は内直筋の後転，外直筋の短縮とともに上下直筋の移植という4直筋を同時に操作する方法が最も多く行なわれている。しかし，4直筋を同時に操作することは，眼球に対する侵襲がかなり大きいと予想され，術後，前毛様体血管の虚血壊死による合併症を起こした症例が1957年にStucchi^5）らによりなされて以来，Forbes^6），Girard^7）などにより追加報告されているが，本邦における報告はみられないようである。今回，われわれは過去2年間（1970年，1971年）に，3名の外直筋麻痺患者に対して，4直筋の同時筋移植を行なつた結果，術後に合併症を起こした2症例を経験したので報告する。

Two cases with intraocular complications of muscle transplantation were reported.

These cases were traumatic 6th nerve para-lysis, and which were treated by three or more muscle transplantation at one stage.

These complications were subacute anterior uveitis, iris atrophy irregular pupill and hypo-tony which appeared postoperatively.

It was already proved by Girard that these complications were due to depriving of blood supply from the anterior ciliary arteries.

Previously, similar complications were report-ed about ten cases which were treated like our cases.
