糖尿病性網膜症に対するVB12の結膜下注射成績

Clinical Appraisal of Subconjunctival Hydroxocobalamin against Diabetic Retinopathy

I．緒言

　最近糖尿病への関心が高まりつつあるが，糖尿病の眼合併症として重要なものに白内障と網膜症がある。白内障は比較的視力の予後が良好であるが，一方網膜症は今日の所，種々の療法によるも改善は少く，進行し，特にインシュリンや新らしい糖尿病治療薬の出現により，患者の生命的予後は好転しているのに反し，網膜症の頻度や重症例の増加の傾向がみられ，今日眼科に於ける治療上の重要問題となつている。

　網膜症に対する療法は薬物療法，食餌療法，手術療法，放射線療法等があるが，中心となるものは薬物療法である。薬剤として血管強化剤，止血剤血管拡張剤，蛋白同化ホルモン，VB₁₂，　C, E，果糖，ATP，チトクローム等が用いられるが，いづれも成績はまちまちで，未だ決定的の治療法が確立されていない。

Fifteen cases with diabetic retinopathy were treated by subconjunctivally applied hydroxocobalamin (Redisol H) twice a week for periods ranging from 1 to 12 months.

1. Out of 15 cases thus treated, the thera-peutic effect was excellent in 1 case, fair in 5, and inconspicuous in 5 cases. In the remai-ning 4 cases the retinopathy progressed in spite of treatment.

2. When the clinical effect was classified according to the stage of retinopathy after Wagener, the therapeutic effect was fair in 3 and inconspicuous in 2 out of 5 cases belon-ging to stage Ⅱ.
