無縫合白内障術後の鈍的外傷により虹彩が消失した1例

Aniridia following blunt trauma in a case after self-sealing cataract surgery

目的：鈍性外傷で無虹彩症になった症例の報告。症例：56歳女性が無縫合白内障手術を受けた。その2か月後に右眼を殴打され，前房出血が生じた。1週間後に視力が1.0に回復したが，無虹彩であることが発見され，受傷から2か月後に当科を受診した。細隙灯顕微鏡検査で虹彩はなく，隅角検査で毛様体突起上に虹彩根部があるのみであった。超音波生体顕微鏡検査で，虹彩根部から索状物が白内障術創近くの角膜裏面に付着していた。結論：外傷で虹彩がその根部で離断し，白内障術創から眼外に脱出し，その後創口が自己閉鎖した可能性がある。

Purpose：To report a case of aniridia due to blunt trauma. Case and Findings：A 56-year-old female received self-sealing cataract surgery. She was hit on her right eye 2months later. When hyphema resolved one week later,she had visual acuity of 1.0 but aniridia was found in the injured eye. When seen by us 2months later,slit-lamp biomicroscopy showed absence of iris. Gonioscopy showed stub-like remnant of the iris anterior to the ciliary process. Ultrasound biomicroscopy showed a strand bridging the iris root to the former surgical wound. Conclusion：The iris would have prolapsed through the corneal wound followed by spontaneous closure of the opening.
