腰椎椎間板ヘルニアに対する手術成績と心理的要因―整形外科患者に対する精神医学的問題評価のための簡易質問票（BS-POP）を用いた検討：前向き研究

Relation Between Surgical Outcomes and Psychiatric Problems in Lumbar Disc Herniation Patients : A Prospective Study

　【背景】本研究の目的は，腰椎椎間板ヘルニアにおける術前の整形外科患者に対する精神医学的問題評価のための簡易質問票(以下，BS-POP)と手術成績との関係を明らかにすることである．

　【対象と方法】腰椎椎間板ヘルニアに対して手術を行った30例を対象とした．対象を術前のBS-POPで正常群と異常群に分けて前向きに比較検討した．

　【結果】BS-POP異常群は正常群と比較して，術後1年時の健康関連QOL尺度SF-36の身体機能(PF)と社会生活機能(SF)得点が有意に低かった．すなわち，BS-POP異常群は正常群と比較して，術後1年時のQOLが劣る．

　【結論】術前におけるBS-POPによる評価は，手術成績不良例をスクリーニングできる可能性がある．

　Background：The Brief Scale for Psychiatric Problems in Orthopaedic Patients (BS-POP) is a questionnaire that was designed to evaluate psychosocial problems in orthopaedic patients.　The purpose of this study was to clarify the relationship between the preoperative BS-POP scores of lumbar disc herniation patients and their surgical outcomes.

　Methods：Thirty lumbar disc herniation patients were the subjects of this study.　The subjects were divided into two groups according to their preoperative BS-POP scores and compared prospectively.

　Results：The group with BS-POP scores in the abnormal range had lower physical function and social life function scores on the MOS 36-item Short Form Health Survey (SF-36) than the group with BS-POP scores in the normal range.　These findings predicted poorer quality of life one year postoperatively in the group with preoperative BS-POP scores in the abnormal range.

　Conclusion：The BS-POP can be used as a preoperative screening tool to predict poor surgical outcomes.
