前方進入法を用いて9例9関節の強直股に対し人工股関節全置換術(以下,THA)を施行した.術後平均追跡期間は,2.6(0.5~6.4)年である.平均手術時間68.7分,術中出血量376ml,術後入院期間13.6日であり,主訴であった腰痛または膝関節痛は軽減し,全例,T字杖による独歩が可能となり退院した.術後5週以内に脱臼が1例発生した.強直股に対するTHAは手術手技の難易度が高いとされているが,前方進入法では,通常の手技手順で正確なインプラントの設置が可能であり,短期成績も良好であった.
We report the clinical results of total hip arthroplasty (THA) by the direct anterior approach (DAA) to treat an ankylosed hip in 9 patients (9 hips). The mean follow-up time was 2.6 years (range, 0.5-6.4 years). Operation time averaged 68.7 minutes, and average blood loss was 376 ml. The mean hospital stay was 13.6 days. All patients were discharged using a T-cane to reduce their low back pain or knee pain. Dislocation in one hip occurred within 5 weeks after surgery. Although THA of an ankylosed hip is a technically demanding procedure, DAA allowed accurate and reproducible component positioning by the usual procedure and has yielded good short-term results.
