　前方進入法を用いて9例9関節の強直股に対し人工股関節全置換術(以下，THA)を施行した．術後平均追跡期間は，2.6(0.5～6.4)年である．平均手術時間68.7分，術中出血量376ml，術後入院期間13.6日であり，主訴であった腰痛または膝関節痛は軽減し，全例，T字杖による独歩が可能となり退院した．術後5週以内に脱臼が1例発生した．強直股に対するTHAは手術手技の難易度が高いとされているが，前方進入法では，通常の手技手順で正確なインプラントの設置が可能であり，短期成績も良好であった．

　We report the clinical results of total hip arthroplasty (THA) by the direct anterior approach (DAA) to treat an ankylosed hip in 9 patients (9 hips).　The mean follow-up time was 2.6 years (range, 0.5-6.4 years).　Operation time averaged 68.7 minutes, and average blood loss was 376 ml.　The mean hospital stay was 13.6 days.　All patients were discharged using a T-cane to reduce their low back pain or knee pain.　Dislocation in one hip occurred within 5 weeks after surgery.　Although THA of an ankylosed hip is a technically demanding procedure, DAA allowed accurate and reproducible component positioning by the usual procedure and has yielded good short-term results.
