背景:人工股関節置換術(THA)後に股関節機能の改善に伴い尿失禁症状が改善したと訴える症例を経験することがある.当院におけるTHA前後での尿失禁症状の変化を前向きに調査した.
対象と方法:当院において前方進入法でTHAを施行した女性133例を対象とし,術前,術後3カ月で,国際尿失禁会議質問票を用いたアンケート調査を行った.
結果:THA術前に尿失禁症状を認めたのは44例(33%)であり,その大半は腹圧性尿失禁症状であった.術前尿失禁有症者のうち25例(57%)で術後に症状の改善を認めた.
まとめ:股関節機能の改善に伴い尿失禁症状が改善する症例が存在することが明らかになった.
Background:We prospectively evaluated subjective aspects of impairment of quality of life by urinary incontinence before and after patients with hip dysfunction underwent total hip arthroplasty (THA).
Methods:The subjects were 133 female patients with hip osteoarthritis who underwent primary THA. We used the International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF) to assess symptoms of urinary incontinence before and 3 months after surgery.
Outcomes:Before surgery 44 (33%) of the 133 patients were incontinent of urine. At 3 months after surgery the urinary incontinence had diminished in 25 (57%) of these 44 patients and resolved in 17 (39%) of them.
Conclusions:These findings suggested a relationship between hip joint function and pelvic floor function, and urinary incontinence resulting from hip dysfunction was suggested by the data.
