 Spontaneous spinal epidural hematomas (SSEHs) cause the sudden onset of paraplegia. Surgical decompression and removal of the hematoma is considered the standard treatment, but some recent reports have recommended conservative treatment. Significant neurological recovery was observed in 7 cases of our 9 cases of acute-onset paraplegia secondary to SSEHs without performing surgery. In this article we summarize our treatment strategy in the form of a kind of algorithm. The indications for surgical treatment depend on the severity of the paraplegia, potential for neurological recovery, and the patient's general condition. Conservative treatment is one of the strategies that is available for the treatment of acute paraplegia secondary to SSEHs.
