AO分類type C橈骨遠位端関節内骨折に対する掌側locking plateの固定法について

Procedure of ORIF with Volar Locking Plate for AO-type C Distal Radius Fracture

　目的：AO分類type C橈骨遠位端骨折を掌側locking plateのみで対処するため独自に分類し手術を行ったので，その治療成績について報告する．対象と方法：手術対象のAO-type C橈骨遠位端骨折43例を対象とした．X線像に基づいて3型に分類し，それぞれに一定の方法で手術を行い臨床評価，X線学的評価を行った．結果：1例を除く42例が掌側locking plateのみで手術可能であった．各型の成績はそれぞれ満足いくものであり，各型間に成績の差を認めなかった．考察：AO-type C橈骨遠位端骨折に対しては，その側面像で分類し，それぞれに合わせた方法で掌側locking plateのみで対応可能であると考えられる．

　Purpose：Clinical and radiological results are reported for AO-type C intra-articular distal radius fractures (IADRFs) that were uniquely classified preoperatively for treatment with a volar locking plate alone.　Subjects and method：43 IADRF cases due for operation were classified into three types based on X ray CT lateral/sagittal views.　These were each treated by a standardized surgical method using a volar locking plate, followed by clinical and radiological evaluation.　Result：Excluding one case where surgery didn't proceed according to preoperative planning clinical and radiological results were satisfactory for treatment by locking plate alone in 42 of the cases.　No significant between-type differences were observed for the three types except for the case.　Conclusions：Evidence suggests that AO-type C IADRFs can be classified by type through Xray CT lateral/sagittal views and then treated with a volar locking plate alone via type-matched surgery.
