6年間の経過で臨床および画像所見の著明な改善を確認した慢性再発性多発性骨髄炎の1例を報告する.症例は初診時9歳の女児であり,左鎖骨に初発し左上腕骨に続発した骨髄炎を認めた.組織学的には両部位とも骨髄炎の所見を呈した.腫瘍性病変の可能性を否定した後,消炎鎮痛剤および抗アレルギー薬の投与を行った.初診後6年の時点で臨床症状および血液,画像所見がほぼ正常に回復していることを確認した.骨成長期の本症例において機能障害が残存するとの報告がみられるが,本症例では機能障害は認めなかった.
We report a case of chronic recurrent multifocal osteomyelitis (CRMO) that showed remarkable clinical and radiological improvement over a period of 6 years. The patient was a 9-year-old girl who experienced pain and symptoms of inflammation, first in the left clavicle, and then in the left humerus. Since a bone biopsy did not reveal any evidence of a neoplastic lesion, her course was closely monitored. Her symptoms repeatedly improved and worsened and ultimately resolved. Six years after her first visit to our hospital, the absence of dysfunction of her left arm was confirmed, and her clinical condition and laboratory and radiological findings had become almost normal.
