　腰椎手術後に発生した椎間関節囊腫による神経根症状に対し保存的治療が有効であった症例を経験した．症例は53歳の男性．腰部脊柱管狭窄症に対し顕微鏡下のL4/5後方除圧を施行後5カ月で左下肢痛が再燃した．MRI，椎間関節造影でL4/5左側に椎間関節囊腫を認め，透視下で穿刺を行い症状は速やかに改善した．腰椎手術後に発生する椎間関節囊腫は，稀ではあるが術後疼痛再発の一因として念頭に入れておくべきである．保存的治療が有効な症例もあり，まずは保存的治療を試みてもよいと思われる．また，透視下での穿刺も有用な方法と考えた．

　A 53-year-old man experienced left leg pain after microscopic decompression for lumbar canal stenosis at L4/5. An MRI examination revealed a cyst around the left facet of L4/5. To treat the cyst, it was punctured under fluoroscopic guidance. The patient's leg pain resolved following the puncture. Although rare, facet cyst should be borne in mind as a cause of symptom relapse after surgery on the spine. Conservative treatment such as puncture should be attempted first.
