腰痛・坐骨神経痛治療アルゴリズムの試み

Algorithm of the Treatment for Sciatica

　抄録：腰椎変性疾患に対する治療方針は同じ病院の中でも各医師によって異なることがある．そこで入院中の治療の流れを1本化し，腰痛・坐骨神経治療アルゴリズムを作成した．アルゴリズム作成にあたっては2年間の入院患者の治療経過を検討し，その結果から治療方針を決定した．アルゴリズムは脊髄造影に始まり，その後には手術あるいは神経根造影・ブロックを行う．ブロックの効果判定の結果，効果があれば外泊しその後退院・手術あるいは再ブロックの選択となる．効果がなければ違う神経根造影・ブロックあるいは椎間板造影・ブロックか椎間関節ブロックを行う．さらにその後も効果によって次の方針が決まっており，最終的に退院・手術あるいは再検討に至るようになっている．アルゴリズムを使用して入院時の説明がよりわかりやすくなり，入院後もブロックや手術の受け入れがよくなってきている．

　We constructed an algorithm of the treatment for sciatica. The purpose of this algorithm is standardization of treatment, which frequently differs from doctor to doctor. This algorithm is based on data gathered over the past two years in our hospital. This algorithm begins in lumbar myelography. From there we can choose the next step；operation or nerve root block. If we choose nerve root block, we perform and judge the effect of the nerve root block. If the block is effective for several days, the patient is permitted to leave hospital. If it is not effective, we perform an other nerve root block, or an intervertebral disc block, or a facet block. If this is effective for a few days, the patient chooses the next step；operation or re-block. After introduction of this algorithm, explanation of our treatment for the patient became clearer and acceptance of the treatment improved.
