下肢人工関節置換術後における深部静脈血栓症の予防効果に関する足底部と下腿部間欠的空気圧迫装置の比較

Comparative Study of Two Intermittent Pneumatic Compression Systems (Foot Pump and Calf Pump) for Prevention of Deep Vein Thrombosis after Total Hip and Knee Arthroplasty

　下肢人工関節置換術後の深部静脈血栓症（DVT）に対するフットポンプとカーフポンプの予防効果を比較した．近位型DVTの発生頻度はフットポンプ使用例とカーフポンプ使用例の間で有意差はみられなかったが，遠位型DVTの発生頻度はカーフポンプ使用例でフットポンプ使用例より有意に低かった．遠位型DVTのうち，下腿本幹静脈血栓症単独発生例に関して有意差はみられなかったが，三頭筋静脈血栓症単独発生例に関してはカーフポンプ使用例がフットポンプ使用例より有意に低かった．すなわち，術後三頭筋静脈血栓症（ヒラメ筋静脈血栓症）の予防にはカーフポンプがフットポンプより有用であった．

　We compared the efficacy of two intermittent pneumatic compression systems, the foot pump and calf pump, in preventing deep vein thrombosis (DVT) after total hip anthroplasty (THA) and total knee anthroplsty (TKA). The incidence of proximal DVT was virtually the same in the foot pump group and the calf pump group, but the incidence of distal DVT was lower in the calf pump group. The incidenee of calf trunk vein thrombosis was the same in both two groups, but the calf muscle venous thrombosis (in soleal vein) was more frequent in the foot pump group. These results indicated that the calf pump is significantly more effective than the foot pump for preventing DVT after THA and TKA.
