反復性膝蓋骨脱臼に対する内側膝蓋大腿靱帯再建術後長期経過した5例の検討

Long-term Results of Medial Patellofemoral Ligament Reconstruction for Recurrent Patellar Dislocation

　対象は反復性膝蓋骨脱臼に対し内側膝蓋大腿靱帯（以下MPFL）再建術を施行後の長期経過を調査できた5例5膝で，手術時平均年齢は30.8歳，術後調査期間は平均11.8年であった．術後成績の評価では，Kujala scoreが術前平均66.2点から術後94.4点に改善し，Crosby and Insall法ではexellent 3膝，good 2膝であった．単純X線像では，全例膝蓋大腿関節の適合性は良好で，変形性変化の進行も見られなかった．今回の結果は本術式の長期にわたる有効性と安全性を示すものと考える．

　The long-term results of medial patellofemoral ligament (MPFL) reconstruction were reviewed. The subjects of this study were 5 patients (5 females, mean age 30.8 years) with recurrent patellar dislocation. The mean follow-up period was 11.8 years. The mean Kujala score was 66.2 preoperatively and 94.4 postoperatively. According to the Crosby and Insall grading system, 3 cases were exellent and 2 cases were good. On axial radiographs, the patellofemoral congruity was good and osteoarthritis had not progressed in all cases. This result demonstrated the long-term effectiveness and safety of the MPFL reconstruction.
