We report a case of stress fracture of the ulnar diaphysis caused by excessive training. A 17-year-old female volleyball player experienced pain in the ulnar diaphysis after her ulna was struck by a volleyball. She was initially diagnosed with a fresh fracture of the ulnar diaphysis, but 4 months conservative treatment resulted in delayed healing of the fracture site. The patient was ultimately referred to our clinic 4 months after the initial diagnosis. A careful patient history and examination of X-ray images indicated that excessive muscle training may have caused a stress fracture of the ulna. The training had required her to stand on her hands in a push-up position with a partner holding her up by the ankles. Low-intensity pulsed ultrasound was used to treat the delayed union of the stress fracture, and complete healing was achieved after 8 weeks. The etiology of the fracture was axial loading during excessive muscle training.
