　66歳の男性が急性に健忘，全身けいれん発作を来し，その後1カ月で，意識障害，失見当識，記憶障害，精神症状，仮性球麻痺，歩行不能，尿失禁が出現した。頭部CTは異常なく，MRIでは大脳白質に散在性にlacunar stroke様の病変を認めるのみであったが，SPECTでは脳内各部に広汎なhypoperfusion領域がみられた。血沈亢進，血清CRP，LDH，γ—globulinの高値，髄液蛋白，細胞の増加を伴った。経過中，発熱，SIADH，副腎腫大，脾腫，肺浸潤，汎血球減少がみられ，神経症状も増悪し失外套状態となり，発症3カ月で死亡した。剖検にてBリンパ球の腫瘍化によるintravascular lymphomatosis（IL）の所見が脳，肺，腎，副腎，脾，膵にみられた。SPECTが本例の病態生理を最も的確に反映しており，SPECTがILの病態把握とその診断に有用であり得ることを述べた。

A 66-year-old male experienced the sudden onset of amnesia and generalized convulsions, and during the subsequent month developed consciousness dis-turbance, disorientation, memory disturbance, abnormal speech and behavior, pseudobulbar palsy, unsteadiness and urinary/fecal incontinence. The initial brain CT scan was unremarkable, and the MRI findings were also unremarkable, except for evidence of several scattered lacunar strokes in thecerebral white matter. ^99mTc-HM-PAO-SPECThowever, revealed multifocal large hypoperfuseclesions in the cerebrum.
