The surgical removal of a pinealoma continues to be one of the most challenging problems for neuro-surgeons and the most popular treatment of this tumor has been shunt operation and/or radiotherapy.

Inspite of the geat surgical risk there is a pos-sibility of total removal of a pinealoma with a per-manent cure. 19 cases of pinealomas have been recently removed surgically with the aid of meticu-lous operative techniques, continuous ventricular drainage, hypothermic anesthesia, administration of corticosteroids and anticonvulsives, parenteral fluid therapy and other careful postoperative treatment.

16 of the patients were discharged in good or fairly good condition, and 3 died before discharge. From these experiences, the surgical removal ac-companied with postoperative radiotherapy would be the most recommendable treatment of pinealoma.
