エリテマトーデスの精神神経症状

Neurological and Psychiatric Symptoms of Disseminated Lupus Erythematosus

　播種性紅斑性狼瘡即ちエリテマトーデス（急性型及び亜急性型）は，皮膚発疹のみでなく，全身臓器を系統的に侵す疾患であつて，膠原病colla-gen diseaseという観点から，またそのL-E現象や血清蛋白の変化を手がかりとして，一般アレルギー疾患の病因論へ近ずく道をあたえるのではないかという点から，最近にわかに問題の中心点となつている（Allen, A．C．）。そしてそれが中枢神経系を侵した場合は，いろいろの精神神経症状を示すのであるが，すべての疾患の場合と同様，精神神経症状を考える時，それがエリテマトーデス独自の中枢神経障害なのか，またはその疾患の身体障害の二次的結果として中枢神経系に障害をひきおこしたものなのか--は区別して考える必要がある。特にエリテマトーデスが，腎，心，肝などを強く侵して，尿毒症や高血圧や全身浮腫などを屡々ひきおこすことを考えれば，この考慮は特に大切となる。即ちエリテマトーデスの精神神経症状という場合には，

　（1）エリテマトーヂス独自の中枢神経系の障害　　　→症状。

Lupus erythematosus (acutus and subacutus) has recently been a center of discussion in various of medicine.

Three cases which showed psychic symp-toms were reported. The first case had a shizophrenic reaction ten years before the skin manifestation. The other two casesdeveloped symptoms such as negativism, change of mood, delusion of reference and agitation after the skin manifestation.

In all three cases psychic symptoms appear-ed prior to ACTH or cortisone administration. The clouding of consciousness which is often remarkable in the organic brain syndrome was not observed.
