 Mental capacity refers to the ability of a patient to give informed consent(IC)and to make autonomous decisions. Although assessing patients' mental capacity is necessary for IC, this is not fully recognized in Japan. As an example, we report on the case of a female patient with schizophrenia who developed catatonia after breast cancer surgery. After recovering from the catatonia this patient reported that she had not expected to lose breast tissue during the surgery. We assessed her mental capacity according to four factors:understanding, reasoning, appreciation, and assertion. We found that the patient had insufficient understanding and appreciation of events to provide IC.
