Gilles de la Tourette症候群の1例—汚言症形成の精神病理に関する一考察

A Case of Gilles de la Tourette's Syndrome: A consideration on the development of coprolalia

　抄録　34歳の離婚歴のある女性で，多発性チックと汚言症を示すGilles de la Tourette症候群の1例を報告し，症状形成に至る経過を詳細に検討して精神病理学的に考察を加えた。

　本症例では，9歳から11歳まで多発性チックの既往があるが，22歳までは症状は消失していた。22歳頃から運動性チックと音声チックが再び出現し，常習飲酒や家族への暴力などもあって精神科に入院した。汚言症の形成過程をみると，反響言語－常同言語の段階を経て無意識的な汚言症が出現したように思われ，同様に運動性チックの症状形成についても，反響行為－常同行為の段階を経て不随意運動が形成されたと考えられた。汚言症と「汚動症」とでも呼ぶべき不随意運動の形成過程には類似の精神病理が認められた。本症例の特徴として，感情的記憶処理の障害と，学習が却って負の効果を生ずる傾向が存在することが指摘され，これが本症例の症状形成に関与しているように思われた。

　A 34 year-old divorced female case of Gilles de la Tourette's syndrome (GTS) with typical symptoms such as multiple tics and coprolalia was reported, and the process of the symptom formation in this case was examined.

　The detailed retrospective investigations on the development of vocal tics in this case revealed that echolalia appeared first, stereotyped vocalization next and coprolalia lastly. Similarly, violent involuntary movements ("copropraxia") seemed to appear in the last stage of the development of motor tics. An analogous intrapsychic mechanism seemed to exist in the process of symptom formation of both coprolalia and involuntary movements.
