心筋硬塞に合併した心破裂の2剖検例—その臨床病理学的検討

Two cases of cardiac rupture following acute myocardial infarction

　急性心筋硬塞後の心破裂は，Power failure（心原性ショック心不全）や，不整脈についで多い重篤な合併症である^1）。特に最近では，死因に占める心破裂の割合が多くなっているといわれ，注目をあつめつつある。一方，心破裂の発生機序に関しては，基本的には硬塞部の軟化度とその部にかかる心内圧との関係で成立すると考えられるが，その詳細についてはまだ不明な点が多い。　今回われわれは，心筋硬塞に合併した心破裂例を2例経験し，いずれも病理的検索をおこなったが，その中で心破裂の発生機序に関して，急性期直後の例と急性期後期（1週間前後）の例で，若干異なる印象を受けたので心破裂に関する一般的特徴とあわせて報告する。

Two cases of cardiac rupture following acute myocardial infarction were clinico-pathologically studied.

The first case was a 71-year-old female dying suddenly 7 days after the onset of acute myocardial infarction. At the autopsy, transmural antero-septal and apical infarct with rupture of left ventricle and complete occlusion with fresh throbus of left anterior descending coronary artery were revealed.
