 Fatal intestinal necrosis caused by nonocclusive mesenteric ischemia(NOMI)developed on 2nd postoperative day after conventional CABG in an 80 year-old man with angina pectoris. He underwent resection of the small intestine. He survived and was discharged on the 19th postoperative day. He had no risk factor for NOMI after cardiac surgery. The cause of NOMI during cardiac surgery is suspected to be a hypovolemic state and much use of vasonstrictive agents such as ephedrine or noradrenaline.

 Survival cases that have developed NOMI during cardiac surgery are rare but early diagnosis and prompt treatment make the survival of such patients possible.
