Tips and Pitfalls for Gastric Cancer Screening by Transnasal Endoscope

要旨●最新型経鼻内視鏡システムにはレーザー光源が搭載され，その画質は通常径内視鏡に匹敵するとまで評価されている．経鼻内視鏡が持つシャフトの柔軟性，小さな曲率半径といった特性を生かした観察を行えば，早期胃癌の診断にも役立つと考えられる．さらに画像強調観察も可能であり，早期胃癌の診断だけではなく，胃癌の原因とされているH. pylori菌感染胃炎の診断支援にもなることが期待されている．最新型経鼻内視鏡は，上部消化管スクリーニング検査において，問題ないスペックを有する機種世代に到達しており，今後は対策型胃がん検診においても必要不可欠な存在になるものと考える．

　A laser light source has been loaded into the most recent model of the transnasal endoscope ; therefore, its image resolution is better than that of conventional endoscopes. A transnasal endoscope has a flexible shaft and a small curvature radius ; hence, it will be useful for the diagnosis of early gastric cancer. Moreover, image-enhanced endoscopy makes it possible to diagnose not only early gastric cancer but also H. pylori-infected gastritis, which is a leading cause of gastric cancer. Transnasal endoscopy has already become extremely popular in UGI screening. We consider that transnasal endoscopy will be indispensable in the screening of population-based gastric cancer in the near future.
