経鼻内視鏡による胃癌スクリーニングの現状と問題点

Present Status and Problems of Cancer Screening by Transnasal Endoscopy

要旨　咽頭反射誘発が軽度である経鼻内視鏡検査は，苦痛が少なく受容性の高いというスクリーニング目的により適していると考えられ，加えて呼吸・循環器系に対する影響もより軽度であり，安全性の観点からも優れている．経鼻内視鏡検査は，経口内視鏡との診断精度の差や，鎮静剤による偶発症に配慮して行えば，安全で苦痛なく施行できる理想的な検査手技であることが明らかとなってきた．今後さらに詳細な検討を要することではあるが，胃癌のスクリーニング検査としては，一定以上の技量の術者が慎重に施行すれば，十分に使用可能であると考えられた．ただし，機器の性能に由来するいくつかの問題点をかかえており，この点について十分留意する必要がある．

　Transnasal endoscopy is considered suitable for screening examination because of the slight degree of the associated gag reflex and fewer adverse effects on the cardiovascular system. Characteristics of transnasal endoscopy were investigated in comparison with conventional endoscopy. Transnasal endoscopy is patient friendly, safer than conventional endoscopy and easier to accept. In addition, as a screening modality for gastric cancer, the sensitivity of transnasal endoscpy is almost equal to other modes for the detection and elucidation of early lesions. However, transnasal endoscopy involves many other problems yet to be solved, including image resolution and poor lighting due to low-caliber light guides. Attention to such problems is thus needed for this new modality of upper GI examination.
