要旨　患者は60歳代，女性．全身倦怠感を主訴に来院し，鉄欠乏性貧血を認め入院となった．上部消化管内視鏡下の生検で，野生型トランスサイレチン（transthyretin ; TTR）由来のアミロイド沈着を認め，老人性全身性アミロイドーシス（senile systemic amyloidosis ; SSA）と診断した．内視鏡像では，十二指腸に多発するびらん・小潰瘍と粘膜と襞の腫脹・断裂を認めた．また，S状結腸にも不整形の多発する浅い小潰瘍を認めた．小腸X線造影検査では，十二指腸から上部空腸にかけて多発する小顆粒状または粘膜下腫瘍様の結節状の小隆起，Kerckring皺襞の腫大を認めた．十二指腸病変からの生検で，粘膜固有層から粘膜下層にかけてのアミロイド沈着がみられ，抗TTR抗体による免疫染色で陽性像を呈した．本症例でTTR遺伝子の解析を行ったが，変異を認めなかったことから，SSAと診断した．SSAで広範囲な消化管病変を呈することは珍しく，本稿では詳細を解説する．

　A 60-year old woman presented with general fatigue and was subsequently admitted to our hospital because of iron deficiency anemia. A diagnosis of senile systemic amyloidosis was made by biopsy specimens via esophagogastroduodenoscopy, which showed amyloid deposits that were immunopositive for TTR（transthyretin）, and genetic analysis revealed the presence of wild-type TTR gene. Endoscopic findings included multiple nodular or fine granular protrusions, small ulcers, and swollen interrupted Kerckring folds in the duodenum. Multiple small shallow ulcers were observed in the sigmoid colon. Results of a barium meal study confirmed multiple submucosal protrusions, small fine granules, and enlarged folds in the duodenum and proximal jejunum.
