Ⅱc型早期直腸癌の1例

Type IIc Early Carcinoma of the Rectum, Report of a Case

要旨　患者は52歳，男性．大腸癌検診でのS状結腸内視鏡検査で肛門縁より14cmの部位に発赤した小隆起性病変を認め，生検組織検査で分化型腺癌が証明された．低位前方切除術を施行したが，切除標本にて病変は直径10mmのⅡc型早期癌で，組織学的には，深達度mの高分化腺癌であった．

　A 52-year-old man had a sigmoidoscopic examination as a part of mass screening program for colorectal cancer, revealing a flatly elevated, small, and reddish lesion at 14 cm from the anal verge (Figs. 1a and b). Biopsy specimen obtained from this lesion proved to contain adenocarcinoma.

　Low anterior resection was performed with extensive lymph node dissection as well.

　The operative specimen revealed type Ⅱc early carcinoma, 10×10 mm in size, arising from the upper rectum (Figs. 3a-c). Histologically it was well differentiated adenocarcinoma in the propria mucosa.
