 Six thousand polyps were removed endoscopically during the past eight years. According to their shape, in addition to endoscopic findings diminutive polyps were divided into four groups (subpedunculated, sessile, superficial and depressed type). A total number of 3,435 diminutive polyps (5 mm or less in diameter) out of these 6,000 were examined histopathologically and morphologically. Neoplastic polyps among the diminutive polyps numbered 2,229 (64.9%). It is very important to identify carcinomas (36 cases, 0.1%) among these diminutive ones. Therefore, colonic diminutive lesions should not be ignored or overlooked because of their small size. These lesions should be removed endoscopically and examined pathologically. These diminutive lesions can be recognized by their slightly different color, and/or uneveness making them stand out from the surrounding mucosa.

 We hope that the origin of colonic carcinoma may be further understood by the accumulation of evidence concerning such diminutive lesions as we have mentioned.
