 Seventy-four patients who had surgical sphincterotomy performed at the Department of Surgery Ⅰ, Kyushu University Hospital, from 1965 through 1970 were investigated to predict long-term results of endoscopic sphincterotomy. Indications for the procedure in these patients were listed in Table 1. Two patients died during the immediate postoperative period. A postal survey was conducted twice in 1981 and 1985 on the remaining 72 patients and endoscopic retrograde cholangiography was performed on selected cases. 53 patients (74%) replied to the 1981-survey; 35 (81%) were asymptomatic, 8 had some complaints and 10 had died. The 1985-survey on the 62 patients who were alive in 1981 yielded an information on 47 patients (76%); 38 (84%) gave a good result, 7 poor and 2 had died (Table2). The cause of death in this series was stroke in 5, hepatoma in 3, stomach cancer in 1, advanced age in 1 and unknown in 2. Three patients with complaints were unlikely to have recurrence of stones or stenosis of the sphincterotomy, since their mild symptom showed no progess during the 4-year interval between the two surveys, and retrograde cholangiography revealed no abnormality in the biliary tract in another. Thus, the number of patients with complaints presumably of biliary origin was only 4 (9.3%) in 1981 and 3 (6.7%) in 1985. The long-term results of surgical sphincterotomy seemed to be better than previously claimed by the creators and promotors of sphincteroplasty. This may give a good and favorable prospect as for the long-term consequences of endoscopic sphincterotomy.
