 Stenosis of the distal common bile duct (papillary stenosis) without. gallstone disease is a distinct clinical entity which is characterized by recurrent attacks of severe pain in the right upper quadrant or epigastrium with or without jaundice. It is usually caused by stenosis of the sphincter of Oddi without other demonstrable pathologic changes in the entire biliary tract. The stenosis may be secondary to either inflammatory changes with or without fibrosis, glandular hyperplasia, muscular hypertrophy, or in some cases to a dysfunctional sphincter with intermittent obstruction. The so-called “postcholecystectomy syndrome” obstructive jaundice, dilatation of the common bile duct, and pancreatitis as well as gallstone disease are the conditions most frequently associated with the various forms of the stenosis of the apilla. Preoperative diagnosis of the papillary stenosis can be made by ERGP with the demonstration of a swollen papilla, dilated common bile duct, and delayed biliary drainage. Endoscopic manometry and electromyography, and a positive morphine-prostigmin test may also be useful for the diagnosis. Symptomatic relief of the patients is usually excellent following abruption of sphincter activity either by transduodenal surgical sphincteroplasty or by endoscopic electrosurgical sphincterotomy.
