短期間（約40日）にX線所見が著しく変化したreactive lymphoreticular hyperplasiaの1例

Reactive Lymphoreticular Hyperplasia of the Stomach Showing Marked Changes of the Radiologic Findings in a Short Period, Report of a Case

　最近，われわれは40日間にX線所見が著しく変化したreactive lypmhoreticular hyperplasiaの1例を経験したので報告する．

　症　例

　患　者：47歳，女．

　主　訴：空腹時心窩部痛．

　家族歴，既往歴；特記すべきものなし．

　現病歴：1973年7月初旬より空腹時に心窩部痛を覚え，同年7月3日に某医を受診した．同医のもとで胃X線および内視鏡の検査を受け，手術をすすめられたがそのまま放置していた．同年11月初旬，上記同様の症状が出現したので，同年11月6日癌研内科を受診した．

　A 47 year-old female patient was admitted to the Department of Internal Medicine, Cancer Institute Hospital on November 27, 1978 under the diagnosis of advanced gastric cancer. She noticed epigastric pain before meals in the beginning of July, 1973. On July 3rd, 1973 radiologic examination was done by a neighboring doctor and gastrectomy was recommended. Refusing the operation, she had been under the medical treatment until the beginning of November,1973 when the epigastric pain reappeared.

　On November 6, 1973 she visited the Department of Internal Medicine, Cancer Institute Hospital. The radiologic examination of the stomach, done on the same date, suggested advanced cancer of the gastric antrum. The second radiologic examination, done 40 days after the first one revealed reduction in the size of the niches and the surrounding filling defects, but was still suggestive of malignancy. Endoscopic examination, done 20 days after the first radiologic examination, revealed two ulcers around the gastric angle and a large erosion on the antrum. Biopsy, done two times after admission, was negative for malignancy. Operation was performed on December 14,1973 under the suspected diagnosis of malignancy. Histological examination disclosed reactive lymphoreticular hyperplasia associated with multiple ulcers of the antrum.
