The requirements for the appropriate diagnosis of invasivity of the lesion by double contrast barium enema are as follows; 1) lateral view of the lesion, 2) pneumatically well expanded double contrast procedure, 3) constant deformity of the lesion, 4) characteristics of the anatomical “place” where the lesion is located. Because of anatomical shape of the colon, the lateral view of the lesion can not always be obtained. The lesion may be identified by the oblique view. Therefore, for the diagnosis of the depth of invasion, we need to analyze the “place” where the lesion is located and lines which form the lesion. In addition, we may have to evaluate the depth of invasion by the frontal view. The central depression, whole shape, form of the margin, nature of the surface, converging folds are important clues to judge invasivity of the lesion. I describe how to analyze lines of deformites by the lesion on the lateral and oblique views, how to diagnose the depth of invasion on the frontal view, and notes for reading images.
